Damages to District Vehicles Report Form
(District Owned or Leased Vehicle ONLY)
SAUSD Employees should complete the following form to report an auto accident while driving a District owned / leased vehicle OR to report damages to a District owned / leased vehicle. This form and other additional procedures must be completed within 24 hours of the accident. *This form is ONLY for SAUSD Employees

Damages to Personal Vehicles or Property: Individuals wanting to report damages to their personal vehicles or property may complete the Liability Claim Form for Damage or Injury (see further below).

If you believe that the District bears responsibility for bodily injury or property damages, we encourage you to reach out to us promptly. We are here to offer our full assistance and support. Our office will conduct a comprehensive investigation into all claims to ensure that educational funds are not unnecessarily diverted from student education.
To file a liability claim for property damages or bodily injury* or for any questions or concerns regarding the liability claim process, please contact us at RiskManagement@sausd.us or 714-558-5856.
