MEDICAL / INJURY REPORT

1.  Print out words of the week (see bottom of the page)

2.  Find an article (from internet, newspaper, journal, magazine)  on one of the words and print out the article

3.  Write an essay

      *Essay must include:

      -Heading

      -Name - Last name then First Name,  Teacher's last name,  PE Period, Student ID#

      -Body of the essay consists of:

      -Topic Sentence - must be at least 2 sentences

      -5 supporting paragraphs, each paragraph must be at least 4 sentences

      -Conclusion / Summary (must be at least 3 sentences)

Essay's will be due on Fridays (teacher will specify which Friday it is due)
