  • Parents can enroll their child to the Head Start Program by completing the Online Application by selecting one of the links below or by downloading the Paper Application from one of the links below. (Only 1st-time and 3rd-year returning students need to apply/re-apply; 2nd-year returning students do not need to re-apply, a staff member will be contacting you once your application is received.) Parents can also pick up a Paper Application at a Head Start Center and return it (along with any required documents) by email, postal mail, or at the drop-off location below:

    PARENTS MUST PROVIDE THE FOLLOWING INFORMATION WITH THE APPLICATION:

    - Verification of parents' income (using the following): 1 month of recent (no more than 3 months old) Pay Stubs, Self-Declaration Form, W-2 Form

    - Copy of child's Birth Certificate

    - Copy of child's Immunization Record

    - Recent Physical and Dental Exams
