If your child needs to be on special medication that has to be administered during school hours, you MUST complete an "Administration of Medication” form that requires information and signatures from a parent and the child's doctor. Even items such as aspirin and cough drops can not be administered without the completed form.  DO NOT SEND ANY MEDICATIONS TO SCHOOL WITH YOUR CHILD WITHOUT THESE FORMS COMPLETED FIRST. All medications must remain in the nurse's office and can only be administered by the school nurse or designated office staff. The forms are in the office and need to be renewed each school year. All medications will need to be picked up at the end of the school year, as well, or they will be disposed of in accordance with state laws. Please do not hesitate to contact the school nurse, Luis Gracia, with any questions. He is available at Esqueda on Thursdays by calling the main office (714)431-1500 and by e-mail at Luis.Gracia@sausd.us
